Community Health Worker REPORTS TO:
CHW Supervisor GENERAL POSITION SUMMARY The Community Health Worker is a full-time position that works to mobilize the community to improve the overall public health of the region.
The primary focus of this position will be building relationships to improve public health.
The Community Health Worker will achieve this objective through close collaboration with local stakeholders and direct outreach to the community as a whole.
Individuals interested in this position should be passionate about improving the health of the community and connecting individuals with potentially life saving services.
The Candidate must have a High School Diploma or equivalent, and must successfully complete Community Health Worker training upon hire.
Two years of general work experience and experience in community outreach and working with underserved and marginalized populations preferred.
A valid Virginia Driver's License and reliable transportation is required.
DUTIES AND RESPONSIBILITIES Community Engagement:
Will work to build relationships throughout the community.
Will establish trusted channels of communication that enable community connections to services that benefit public health, financial stability and education to be developed.
Access to Services:
Improve pathways for individuals to access services offered by the United Way and other services providers.
Public/Community Relations:
Work to serve as a conduit from the community to the United Way and other service providers, establishing trusting relationships and communication pathways.
Strategic Planning:
Will develop strategies that mobilize the community to improve public health in Martinsville and Henry County.
KNOWLEDGE, SKILLS, ABILITIES Ability to communicate to individuals and groups of all social classes Experience in developing relationships with organizations and individuals Ability to build consensus and motivate stakeholders to achieve results Strong interpersonal skills Experience working with volunteers D.
